Use this sheet for one scholarship route and one university. The current embassy or university notice controls the actual requirements.

Identity and academic records

• Passport and a copy that meets the notice’s validity requirement.
• Highest degree certificate or proof of expected graduation, if accepted.
• Academic transcripts and any required translations or authentication.
• Language test or medium-of-instruction evidence accepted by the program.

Application documents

• Study plan or research proposal tailored to the program.
• Recommendation letters in the format and number requested.
• Supervisor or pre-admission letter, if the route requires one.
• Foreigner Physical Examination Form, if required for this stage.
• Police clearance or non-criminal record, if required.
• CSCA score for undergraduate routes, if the current notice requires it.

Before submission

• Check the correct route, agency number, portal order, and deadline in the current notice.
• Save a copy of each submitted form and confirmation receipt.
