CSC Physical Examination Form: Complete Guide

1.What is the Foreigner Physical Examination Form?

The Foreigner Physical Examination Form (外国人体格检查表) is a standardized medical form designed by the Chinese quarantine authority. Official CSC guidance states applicants planning to stay in China more than 6 months must upload a photocopy of the completed form — in English — with their online application, and keep the original (Chinese Embassy CGS notice).

It applies to every CGS category: Bachelor's, Master's, Doctoral, and general/senior scholars.

Download it from official sources only: csc.edu.cn, campuschina.org, or an official form PDF mirrored by a university. Using an old or altered version is a known rejection cause.

2.What tests does it include?

The form covers:

  • Medical history checklist for infectious diseases (hepatitis, typhoid, dysentery, etc.)
  • Full physical exam — blood pressure, vision, skin, heart, lungs, abdomen, nervous system
  • Chest X-ray (report must be attached)
  • ECG (the form prints "ECC" — a typo on the official form itself; it means electrocardiogram)
  • Blood tests including HIV and syphilis serology (lab reports must be attached)
  • Physician's conclusion, signature, exam date, and the hospital's official stamp

3.The 5 rules that make or break the form

1. Valid for only 6 months from the exam date — time it so it's still valid at application upload AND enrollment. 2. Photo must be stamped: your photo goes on the form with the hospital's official seal stamped across it. An unsealed photo makes the form invalid (Beijing official rules). 3. Public hospital: for verification in China the exam must be done at a public hospital; private-clinic forms require local notarization first (same source). 4. In English (or Chinese) — CSC asks for English. 5. Keep every original report: X-ray film/report, ECG printout, lab reports — you'll need them again in China.

4.When do you actually need it?

  • At application: uploaded as a scan with your CGSIS/campuschina.org application.
  • At the visa stage: the official X1 visa document list does not include it, but some embassies ask anyway — check your local Chinese embassy.
  • After arrival: within your first weeks your university sends you for health-check formalities. If your overseas form is complete with original reports attached, it can simply be verified (~60 RMB in Beijing) instead of repeating the full exam (~430 RMB). Incomplete forms mean re-testing (Beijing source above).

5.What does it cost?

No official worldwide price list exists. Community-reported ranges (unofficial, varies by hospital):

  • Pakistan: $30–60
  • Bangladesh: $30–50
  • Nigeria: $50–100
  • Egypt: $40–70
  • India: $30–60

6.Common mistakes that get forms rejected

Per official CSC and Beijing rules, forms are commonly rejected for:

  • Missing physician signature
  • Missing hospital stamp
  • Unsealed photo
  • Missing exam date
  • Expired (>6 months)
  • Missing attached lab/X-ray/ECG reports
  • Unclear scans
  • Wrong form version

7.Is the CSC physical examination form required at application or after selection?

At application. CSC requires the completed form uploaded with your online application if you'll stay in China more than 6 months. Keep the original — it's checked again in China.

8.How long is the form valid?

6 months from the exam date. Time it so it's valid both when you apply and when you enroll.

9.Can I do the exam at any hospital?

Use a public/government hospital. Private-clinic forms need local notarization to be accepted for verification in China.

10.Do I repeat the medical exam in China?

If your form is complete with original X-ray, ECG, and lab reports attached, it's usually just verified. Missing items are re-tested at your cost.
